When Can You Change Coverage?
Once you choose your benefit options, you are eligible to make changes or additions during the next annual open enrollment. If you initially decline coverage you must wait until the next open enrollment period to enroll.
However, if you experience one of the following “Qualifying Life Events,” you may be able to enroll or make changes to your existing benefit elections at the time of the event. You must contact Benny’s Café within 31 days of the event in order to change
your enrollment options.

Note: Financial hardship is not considered a qualifying life event for cancellation or changes to coverage.
If any of these events occur and you elect to change your coverage, please notify Benny’s Cafe and complete a Change Form within 31 days. This 31-day eligibility period applies to any change in coverage (such as the addition of a dependent, etc.). You must supply written documentation, including a completed Change Form and the appropriate document from the acceptable documents list in the Dependent Verification Criteria (for example, a birth certificate for a new child), for any qualifying life event. You may also complete a "Qualifying Event Change Form" and fax to Benny's Cafe with supporting documents within 31 days of the event.
