Follow the steps to search for an Anthem PPO Provider near you:

1) Select the Doctor/Medical Professionals or Hospital and Facilities options
2) Enter a doctors name, or a specialty
3) Enter the distance you are willing to travel, and your zip code

On step 4 please follow these steps:

Select “I am a current member and want to search using my plan” OR “I want to search by selecting a plan”

If you search by your plan you will need to use the first three letters of your Member ID Enter alpha prefix OR

Select the State Select

PPO Plan Type/Network option

Select Blue Cross PPO (Prudent Buyer) – Large Group

And click on Search button
